Clinical and immunological characterizations of the disease with long-term follow-up and treatment options have not been previously reported in large cohorts. We sought to determine the clinical and immunological features of CARMIL2 deficiency and long-term efficacy of treatment in controlling different disease manifestations. Methods The presenting phenotypes, long-term outcomes, and treatment responses were evaluated prospectively in 15 CARMIL2-deficient patients, including 13 novel cases. Lymphocyte subpopulations, protein expression, regulatory T (Treg), and circulating T follicular helper (cT(FH)) cells were analyzed. Three-dimensional (3D) migration assay was performed to determine T-cell shape. Results Mean age at disease onset was 38 +/- 23 months. Main clinical features were skin manifestations (n = 14, 93%), failure to thrive (n = 10, 67%), recurrent infections (n = 10, 67%), allergic symptoms (n = 8, 53%), chronic diarrhea (n = 4, 27%), and EBV-related leiomyoma (n = 2, 13%). Skin manifestations ranged from atopic and seborrheic dermatitis to psoriasiform rash. Patients had reduced proportions of memory CD4(+) T cells, Treg, and cT(FH) cells. Memory B and NK cells were also decreased. CARMIL2-deficient T cells exhibited reduced T-cell proliferation and cytokine production following CD28 co-stimulation and normal morphology when migrating in a high-density 3D collagen gel matrix. IBD was the most severe clinical manifestation, leading to growth retardation, requiring multiple interventional treatments. All patients were alive with a median follow-up of 10.8 years (range: 3-17 years). Conclusion This cohort provides clinical and immunological features and long-term follow-up of different manifestations of CARMIL2 deficiency.</subfield> </datafield> <datafield tag="650" ind1="1" ind2="7"> <subfield code="2">opendefinition.org</subfield> <subfield code="a">cc-by</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Baser, Dilek</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Akcam, Bengu</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Danielson, Jeffrey</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Eltan, Sevgi Bilgic</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Erciyes Univ, Dept Med Biol, Sch Med, Kayseri, Turkey</subfield> <subfield code="a">Haliloglu, Yesim</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Sefer, Asena Pinar</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Babayeva, Royale</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Akgun, Gamze</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Charbonnier, Louis-Marie</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Harvard Med Sch, Boston Childrens Hosp, Div Immunol & Newborn Med, Boston, MA 02115 USA</subfield> <subfield code="a">Schmitz-Abe, Klaus</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Univ Hlth Sci, Umraniye Teaching & Res Hosp, Genom Lab GLAB, Istanbul, Turkey</subfield> <subfield code="a">Demirkol, Yasemin Kendir</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Zhang, Yu</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Regeneron Genet Ctr, Tarrytown, NY USA</subfield> <subfield code="a">Gonzaga-Jauregui, Claudia</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Heredia, Raul Jimenez</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="a">Kasap, Nurhan</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Istanbul Univ Cerrahpasa, Fac Med Pediat Allergy & Immunol, Istanbul, Turkey</subfield> <subfield code="a">Kiykim, Ayca</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Istanbul Univ, Istanbul Fac Med Pediat Allergy & Immunol, Istanbul, Turkey</subfield> <subfield code="a">Yucel, Esra Ozek</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Erciyes Univ, Sch Med Pediat Hematol & Oncol, Kayseri, Turkey</subfield> <subfield code="a">Gok, Veysel</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Erciyes Univ, Sch Med Pediat Hematol & Oncol, Kayseri, Turkey</subfield> <subfield code="a">Unal, Ekrem</subfield> </datafield> <datafield tag="700" ind1=" " ind2=" "> <subfield code="u">Erciyes Univ, Sch Med Pediat Hematol & Oncol, Kayseri, Turkey</subfield> <subfield code="a">Unal, Ekrem</subfield> </datafield> <datafield tag="980" ind1=" " ind2=" "> <subfield code="b">article</subfield> <subfield code="a">publication</subfield> </datafield> <datafield tag="542" ind1=" " ind2=" "> <subfield code="l">open</subfield> </datafield> <datafield tag="100" ind1=" " ind2=" "> <subfield code="a">Kolukisa, Burcu</subfield> </datafield> <datafield tag="260" ind1=" " ind2=" "> <subfield code="c">2022-01-01</subfield> </datafield> <controlfield tag="005">20221007091627.0</controlfield> <datafield tag="909" ind1="C" ind2="O"> <subfield code="o">oai:aperta.ulakbim.gov.tr:235246</subfield> <subfield code="p">user-tubitak-destekli-proje-yayinlari</subfield> </datafield> <datafield tag="856" ind1="4" ind2=" "> <subfield code="z">md5:c1a1e64ba2b8e25122f7b16dff885a67</subfield> <subfield code="s">384</subfield> <subfield code="u">https://aperta.ulakbim.gov.trrecord/235246/files/bib-358244b9-5f0b-461d-9059-26b7f13378a9.txt</subfield> </datafield> <datafield tag="540" ind1=" " ind2=" "> <subfield code="u">http://www.opendefinition.org/licenses/cc-by</subfield> <subfield code="a">Creative Commons Attribution</subfield> </datafield> <datafield tag="024" ind1=" " ind2=" "> <subfield code="a">10.1111/all.15010</subfield> <subfield code="2">doi</subfield> </datafield> </record>
